AHA's intensive interactive Health Care Transformation fellowship provides senior health care leaders with the implementation skills to help their organizations execute delivery and payment system transformation. The four focus areas of the program are physician relationships, population/community health partnerships, partnerships with payers and public health organizations and navigating financial risk. Through a combination of in person learning retreats and webinars, fellows leave the program with the tools necessary to implement these new care models.

Objectives of the fellowship
- To educate leaders about key issues in the implementation of new care models to achieve higher quality care and improved efficiency.
- To develop leadership skills in designing, implementing and evaluating new care and payment delivery models.
- To develop relationships between peers and organizations involved in health system transformation.

Value of the fellowship
The fellowship is specifically designed to give participants actionable items to help their organizations effectively execute and implement these care delivery models:
- Action Plans— A synthesis of the main take-aways, key questions to address with their senior leadership team, common challenges and helpful hints for success at the end of each learning retreat.
- Peer-Group Networking— The chance to network with current and former fellows and with faculty from organizations that have implemented innovative care delivery models.
- Pay-it-Forward—The opportunity to continue to build the AHA Health Care Transformation fellows alumni network and to participate as faculty in future classes.
