Lexington Regional Health Center - Culture of Safety

Lexington Regional Health Center, a 25-bed facility in Lexington, Nebraska, used best practices and peer-to-peer learning to reduce harm across the board. By focusing on data collection and presentation, creating a strong team culture where everyone can speak up and identifying patients who need additional follow-up after discharge, the hospital reduced readmissions by 40 percent and decreased patient falls by 70 percent. Lexington Regional Health Center was part of the AHA/HRET Hospital Engagement Network.
