Midstate Medical Center, a 134-bed facility in Meriden, Connecticut, committed to reducing harm across the board by focusing on transparency across all disciplines and levels of care. The hospital's board is heavily involved in this process, and all meetings start with patient safety stories. Audits are done by front-line staff and reviewed with senior leadership. Midstate reduced overall harm across the board by 42 percent. Significant results were achieved by reducing readmissions. Midstate Medical Center was part of the AHA/HRET Hospital Engagement Network.
