Checklists, Frameworks and Guides for Transforming Care

Improving patient safety, leading multiple improvement efforts and moving to value-based care: The AHA’s Hospitals in Pursuit of Excellence initiative supports hospitals’ and health care systems’ efforts in these areas with resources created to accelerate performance and quality improvement and care delivery transformation. For example, “Checklists to Improve Patient Safety” has checklists for 10 areas of patient harm including adverse drug events, early elective deliveries, injuries from falls and immobility, hospital-acquired ulcers, preventable readmissions and surgical site infections. “Leading Improvement Across the Continuum: Skills, Tools and Teams for Success” presents two frameworks to help hospital and care system leaders design and implement a wide range of improvement projects--from the microsystem level to community health level—and maximize project impact. HPOE resources on the second curve of health care include a road map, metrics and reports that outline must-do strategies and organizational competencies.
