NYU Lutheran Healthcare, Brooklyn, N.Y., created a shared medical appointment program transforming the usual doctor visit and providing essential health education to its patients. The program is a 90-minute appointment involving a group of patients and a health care team. Patients still receive necessary prescriptions or orders for tests, but they also have the opportunity to spend more time with their health providers and learn from other individuals with the same health condition. The program is offered to diabetic patients and expectant mothers.

This case study is part of the NYS Triple Aim series highlighting how New York hospitals are improving health, enhancing quality and reducing costs. Hospitals, nursing homes, and home care agencies across New York State are pursuing the Triple Aim. In spite of fiscal constraints, its members are embracing the challenge of transforming health care and are implementing new and innovative approaches to delivery.

NYU Lutheran Healthcare was also featured in a recent HPOE guide, "Diversity in Health Care: Examples from the Field."
