St. Joseph's Hospital of Elmira
St. Joseph's Hospital of Elmira implemented a comprehensive antibiotic usage program beginning March 2010 to enhance patient safety and improve patient outcomes. A multidisciplinary team, also known as the "Bugs & Drugs Committee," completed a gap analysis and formulated an action plan. This plan included an electronic report of all active patients on antibiotics and associated cultures and sensitivities. This report, along with patient chart review and weekly rounds with the infectious disease physician, enables the clinical pharmacist to understand the efficacy of antibiotic regimens.
