Hospitals and health care systems are increasingly focused on finding better ways to deliver care for high-need, high-cost patients. Many of these patients have complex medical and nonmedical needs and require greater support to manage those needs.
This issue brief provides an overview, strategies and resources to help hospitals improve care for high-need, high-cost patients.
This resource library includes case studies, toolkits, guides and best practices from hospital and health system leaders across the country. These resources are classified to address the top 10 clinical categories representing the most prevalent diagnosis-related groups (DRGs), based on 2014 Medicare claims submissions. These categories and resources are still evolving and will be updated to help hospitals improve care for high-need, high-cost patients.
