Huddle for Care

In 2015, the Gordon and Betty Moore Foundation funded HRET to strengthen care transitions by creating Huddle for Care, a website designed to foster sharing and learning between individuals in the care transitions field. The website features stories and effective tools related to care transition topics. The website's discussion forum is a great opportunity to express opinions, share ideas and receive feedback from transitional care program implementers and other website members. The project aims to build a care transitions community within and beyond local areas as well as nurture mutual support and collaboration across programs throughout the country.
