Strengthening the System of Care for Youth and Families
The School Based Care Coordination Program supports at-risk youth and families in two school districts staffed by 2 full-time school based care coordinators and 3.5 full time clinical social workers. SBCC is embedded in the school culture and physical environment and functions as an extension of the school's guidance departments. SBCC provides hands-on care management for at-risk youth and families, on-site clinical counseling services and referrals to community-based resources including health care providers and a host of community-based supports and services.
