Improvement Across the Continuum
Eliminating hospital-acquired infections, preventing patient falls, reducing preventable readmissions, reducing community asthma rates—these are just a few of the many improvement efforts that hospital and health care system leaders are tackling. A Hospitals in Pursuit of Excellence guide “Leading Improvement Across the Continuum: Skills, Tools and Teams for Success ” presents two new frameworks for designing and implementing a wide range of improvement projects and maximizing project impact. The guide outlines the improvement continuum with four levels that build on one another—topic or microsystem/unit, care coordination, defined population and community health—and describes the strategies, skills, tools and teams needed. The leadership action model provides a framework for using the improvement continuum.
