Pressure Ulcer Reduction using Lean Six Sigma

Lean Six Sigma tools were used to reduce pressure ulcers at Memorial Health System, Springfield, Ill. Bundles and communication tools were employed.

The hospital set a goal to reduced pressure ulcers by 50 percent by applying Lean Six Sigma methodologies. Process defects were identified for pressure ulcer prevention, including identifying at-risk patients, initiating interventions, achieving minimal turning requirements and effective care coordination across units and shifts. Key improvements include:

  • Development of a "PUP Kit" consisting of a bundle and bedside communication tools;
  • Creation of a "team-up to turn" approach to achieve minimal turning requirements with support tools; and
  • Enhanced role definition and training for unit-based wound ostomy champions.

Hospital-acquired pressure ulcer prevalence rates for Stages I-IV ulcers have decreased 79 percent, exceeding the initial goal. Read the whole case study below (click "view item").
