Garfield Medical Center - Reducing Readmissions

Garfield Medical Center, a 210-bed facility in Monterey, California, used best practices and peer-to-peer learning to reduce harm across the board. By focusing on patient care, working with the community and increasing staff engagement, the hospital has prevented 568 harms and reduced readmissions by 6 percent. Garfield Medical Center was part of the AHA/HRET Hospital Engagement Network.
