Physician Visits After Hospital Discharge: Implications for Reducing Readmissions

About 1 in 12 adults (8.2 percent) aged 21 and older discharged from a hospital to the community was readmitted within 30 days and 1 in 3 adults (32.9 percent) was rehospitalized within one year of discharge. The implication is that many patients discharged from hospitals continue to be at-risk for readmission beyond the 30-day window that is usually the focus of post-discharge interventions.
