Advanced Care Planning in the ICU

Many Medicare patients with life-limiting illnesses had no documented advanced directive while in the ICU. The hospital’s aim is to address their physical, emotional and social needs by having a plan of care goal conversation in order to establish a written advanced directive. This project initially targeted Medicare patients in the ICU. It grew to include adult patients with advanced, serious illness in all areas of the hospital

This case study is part of the Illinois Hospital Association's annual quality awards. Each year, IHA recognizes and celebrates the achievements of Illinois hospitals in continually improving and transforming health care in the state.
