Using Culture of Safety Survey as a Roadmap to High Reliability

As identified in its 2014 Culture of Safety survey, the hospital’s overall mean score for the standard questions was 64.0, which was 0.4 points higher than the prior year. However, when compared to the all facility benchmark for the standard questions score, the organization was 2.4 points below, leaving room for improvement. The goal was 66.4 points.

Although 19.7 percent of respondents were "most positive" in their survey responses, the organization strove to exceed the benchmark mean score for that standard of the Culture of Safety survey. By meeting this challenge, the organization demonstrated its continued commitment to providing safe, patient-focused care.

This case study is part of the Illinois Health and Hospital Association's annual Quality Excellence Achievement Awards.
