Identifying discrepancies in electronic medical records through pharmacist medication reconciliation.
This study found that pharmacist-led medication reconciliation in primary care settings identified a significant number of discrepancies between charted medications and actual medications being taken. The most common discrepancies were an incorrect medication documented in the chart, often from patients not reporting over-the-counter medication use.
