Quality of Life Index

The Wisconsin Quality of Life Index is a multidimensional model for measuring patients’ quality of life. It takes into account the different perspectives of the patient, the patient’s family, and the clinician, both currently and over time. The W-QLI website provides five instruments, an assessment manual, coding and scoring books and validation papers.

The following instruments are available to download:
