Profiles in Excellence: Quality Improvement Lessons from the AHA-McKesson Quest for Quality Prize Recipients Part 2

The AHA-McKesson Quest for Quality Prize honors hospitals that demonstrate progress in achieving the Institute of Medicine's six quality aims – safety, patient-centeredness, effectiveness, efficiency, timeliness and equity. This webinar explores two hospitals' decisions to look beyond their own organizations to improve quality in their communities. 

Duke University Hospital, Durham, N.C., engages patients and families in their care as well as in hiring and facility design. Duke also collaborates with local health agencies to share information in their electronic health records, design care plans for complex patients and increase access to primary care.

Schneck Medical Center, Seymour, Ind., works closely with local nursing homes to reduce readmissions. By hiring a medical director and nurse practitioner to manage the continuum of care and educate LTC providers, Schneck reduced its readmissions by 40 percent in three years. The organization has also teamed up with a competitor to launch population health initiatives.

Specifically, this webinar focuses on continuous quality improvement strategies and forming partnerships with patients, families, local agencies and even competitors.
